Transitioning back to home is an important milestones in every patients recovery process.  Our thorough assessment evaluates the patients environment, skills, abilities, and needs to determine necessary modifications for a safe living environment.

After the assessment we will help provide budget conscience options through our preferred network of home modifications. We understand change be a lot to manage, and are available for a post home modification session to help  you become familiar with your newly adapted home.
